To enroll your child at The Academy of Future Leaders your child must be between the ages of
6 weeks and 12 years old

The following forms must be completed to reserve your child’s spot at The Academy of Future
Leaders:
o Completed Enrollment Application Packet
o Annual registration fee $125.00 in cash non–refundable
o Bi–weekly tuition in cash or paid online via myprocare.com or on the parent app
o Child’s Medical Information and Form
o Parent Contact Information Form
o Immunizations Record on Tennessee Form
o Childcare certificate if applicable
